The present case is one of the most central in this study. The material was gathered over the course of intensive psychoanalysis through a period of four years. The analysis was conducted on a five-session-per-week basis and was based throughout on more or less classic notions of psychoanalytic therapy. The patient was treated on the couch, and the method of free association and interpretation was employed. It must, therefore, be appreciated that this case is based on material that is both considerably more detailed and voluminous and in many respects qualitatively different from that of the preceding cases. These differences between a more traditionally psychoanalytic and a psychotherapeutic approach, however, offer us an opportunity to inquire into the paranoid process from a somewhat different perspective—one that lends considerable support to the impressions and tentative hypotheses that have emerged in our study to this point.

An important emphasis that the material from this patient provides—as well as the following patient, who was also treated in intensive psychoanalysis—is that the paranoid process operates in significant ways even in patients who are relatively healthy and who can be regarded as potentially good candidates for successful psychoanalysis. The personality structures of the previous patients in this series have all been borderline or psychotic. It was quite clear, however, that Jim possessed a well-established and solidly neurotic character structure. The study of his case, therefore, is particularly interesting in that the paranoid process here displays itself in a much better integrated picture of personality function than we have had opportunity to see to this point.
