The Attack

Ann's intense paranoid struggles with her mother continued through the course of treatment. Mother was constantly involved-intrusively telling Ann what she should do, criticizing her for not doing what the mother thought she should do. Ann interpreted her mother's intrusiveness as picking, as trying to hurt and destroy her, as trying to drive her crazy. Her struggle with the family came to a head soon before her second hospitalization. She started answering her mother back and the mother hit her. Ann started to hit back, and at that point the brother and father jumped in and restrained both the mother and daughter. Ann saw this as a militant attack on herself. She interpreted the intervention of her father and brother as their defending her mother. For her it represented proof that the whole family was against her and that they wanted to hurt her. She saw the intervention not as a preventing of further violence, but as an attack directly on her. We shall see similar critical events in the course of some of our other patients histories. An event occurs which gives substance to the patient's evolving belief, not only that the malicious member of the family is trying to hurt the patient, but that the rest of the family is in collusion against the patient. Such events are often taken by the patient as the critical bit of evidence which proves and clinches the paranoid delusion. They come to carry a burden of conviction far beyond their merit.

For Ann, this provided a critical focus for her belief that the family was trying to hurt her—as well as a verification of that belief. When we discussed this episode in therapy,I tried to point out that perhaps her father and brother were only trying to protect her from being hurt. She responded angrily to this suggestion—asking in great consternation how I could try to defend their action. They had so obviously tried to kill her, and I was trying to defend them. Her conclusion was that I must in some way be conspiring against her and that I also wanted to kill her.
